Job Summary:
- Contacts referred families to
explain the program and engage families into services;
- Conducts behavioral and
developmental assessments with new families;
- Develops service plans with
families base on their assessments and concerns;
- Provides home visitation
services to families weekly to monthly, based on need;
- Oversees and assist with group
meetings, including training and support;
- Regularly provides screenings
for children on caseload;
- Makes appropriate referrals to
government and community agencies for needed assistance;
- Represents the program to the
community, particularly potential referral sources;
- Reviews all cases with
supervisor within 30 days of assessment;
- Documents all contacts in the
case record according to AzPaC and contract guidelines;
- Actively participates in
scheduled staff and team meetings;
- Collaborates with other
involved professionals and family members to maximize effectiveness of the case
plan and amend as needed;
- Reports suspected child abuse
or neglect to Child Protective Services;
- Closes case when a)targeted
goals have been satisfactorily addressed, b) the client requests case closure,
or c) provision of service can no longer achieve goals.
Education/Experience:
- Bachelor degree in early childhood
development, education, family studies, or related human service field, or staff
member is extensively trained and can demonstrate competency in service
provision;
- Prior experience in early childhood
required;
- Experience in family centered practice
and case management preferred.
Special Requirements:

Fingerprint clearance
required;
·
Must have valid Arizona
driver?s license, reliable transportation and current automobile liability
insurance;
Bilingual (Spanish speaking) required.
Schedule/Salary Info:

Full time, 40 hours per week

Pay Grade 11
