Case Management

Our case managers empower our Emergency Shelter, Transitional Housing and Permanent Supportive Housing families to help them achieve a stable life in permanent housing. We create customized case plans with them rather than for them. We show them how to develop their own goals and to build their network and resources to attain those goals.

We believe in helping our families set realistic goals to build their self-esteem and seek new, more far-reaching goals. In addition, our case managers work directly with clients to meet their needs or refer them to agencies more versed in the area of need. They follow up on referrals and transport clients when necessary to ensure there are no barriers to receiving services.

Very often, our case managers will accompany clients to their service appointments to advocate on their behalf. Our clients learn by example how to advocate for themselves in the future. They become familiar with requirements and paperwork so they are not turned away because they did not understand the process or feel comfortable speaking up for themselves.

Our case managers provide transportation vouchers and, in some cases, personal transportation for our clients to health appointments and job interviews. Above all, they are truly compassionate and treat all clients with dignity.
