Shelter Case Manager

DUTIES & RESPONSIBILITIES:
• Provide direct client services as requested in person, by phone, or in writing according to professional standards of all agencies involved.
• Provide case management in Transitional and Emergency Shelter.
• Advocate for clients as needed.
• Assess visibility of services and utilizes community resources.
• Establish and maintain healthy relationships with the community.
• Prepare and maintain accurate up-to-date listing of all known community resources.
• Prepare reports and records as required by CCF and relevant contractors.
• Participate in the process of ongoing personal and professional development.
• Perform any other job-related tasks deemed necessary and/or as assigned by supervisor.
QUALIFICATIONS/REQUIREMENTS:
• Must have knowledge of good interviewing skills.
• Must be knowledgeable of local and state resources available to meet client problems.
• Must have knowledge of appropriate contact guidelines and requirements.
• Bachelors Degree, preferably in social work or related field.
• Experience working with the homeless preferred.
• Knowledge of community resources is important.
• Driving is required; therefore must have reliable transportation and valid AZ Driver’s License,registration,and liability insurance.
• Fingerprint clearance, or eligibility for fingerprint clearance required.
SALARY: Grade 9 position. $11.73/hour
